You're probably running in fullscreen windowed mode and OS capping your refresh rate to desktop refresh rate. Try changing to fullscreen mode (turn on vsync too) and software cap should be removed.

Do you have 60 FPS cap in lobby only? There is a check box in game settings that's limiting lobby FPS to 60, so your GPU won't be 100% loaded while in menu/stash.

Interesting. I'd suggest to go into your Nvidia control panel - Manage 3d settings - global settings - and set Vertical Sync to use 3d application settings. And then relaunch the game and turn on vsync in fullscreen mode. Restart of the game might be required, so FPS cap will go away :)

P.S Also screenshot from your Nvidia control panel - Adjust desktop size and position menu would be helpful. Issue might be related to scaling.
