I moved from Wisconsin to Maui about 2 years ago. The ping change is definitely something you just have to get used to an extent. Going for blocks you gotta assume the opponent's touch will actually come a bit earlier than what it looks like and you're likely to get faked easier now. It's still very playable, at least for me, but definitely hinders you a bit.
I've tried a few regions like Oceania or the East/SE Asias, but never had better luck than NA-West. When I had Spectrum I would get like 80 ping at best, but switched to Hawaiian Telcom fiber and get 55 or so on the best servers now. Tournament servers can be relatively unplayable at times though (105-160). Haven't really had the issues you mention with random latency too much unless it's a server where everyone is experiencing that.
