It's a bug that was introduced in the most recent update. There is a planned fix for it in the next update.

The one on the left is the Legacy version from before f2p and the one on the right is the Alolan form

do we see these report pop ups from all types of bans?

No, not all reasons will pop up just because of how our report and feedback system was originally built. We have made some changes and most recently players who report others for Unsportsmanlike Conduct should see feedback popups

What about the actions that led to the ban?

“Your reports of a player throwing/griefing a match and being intentionally idle has led to a ban.”

We can't and don't want to give any indication of who you might have helped get banned as it opens that person to potential harassment. By providing the actions that led to the ban to the reporter it can pretty significantly narrow down who might have received the ban.

I wish they'd tell me who my reports helped to ban.
