What happens if the last player to go down hasn't used it yet? Is there a grace period for them to activate it or does the game just end?
If any of the players has an unused Iron Will there is a grace period yes.

Wording makes it seem like you perma-die and cant be revived. Probably just me being a leaflover and misunderstanding.

So are there no more tiers/levels of perks? Is it just one level of each perk?
Still tiers but the number of tiers for some of the perks has been reduced mostly based on having nice upgrade steps. Also with most perks the secondary effects (if any) are now available at tier 1.
Do you have to use Iron Will within a certain amount of time from death under normal circumstances?
Yes, I you have about 10 seconds to make the choice.
So you can't go down and wait 1 or 2 minutes before deciding to use it? If you don't use it within 10 seconds you are down until revived?
