It's possible either a typo is on the form (we have a list of all data that was input incorrectly from all channels and there is a good chunk from here), or something else has happened. If it's been over a week, contact support: support.fallguys.com and be sure to tell them your Support Code and where you submitted the survey from (Reddit/Twitter/Instagram)!
That goes for all users who are experiencing issues, we have a log of every single crown awarded and can use it to cross-reference.
