[2019] Can't fire/attack after activating VATS?

I've nearly 100 hours into this game and have not successfully used VATS once. When I press the attack button, it just cancels out. I've heard that this should happen when AP is too low, but I have full AP, weapon is loaded, and stationary, fully exposed target has 95% hit chance. What am I doing wrong? I'm on PC.
