When an item is being vaulted it’s being replaced with one bandage in item pool before removing it. That’s why often after a patch you can see purple or gold bandages in vending machine that will only give you one when you’ll buy it.

Close - internally we test with unreleased items in the loot pool, this is actually a safety net we use to prevent them from dropping in the live game.
Any time you see 1 Bandage in loot... it means we didn't catch the issue in the data. Feel free to report it and we'll fix it :-)
Vaulted items are just removed from the loot pool.
