We were playing with my friend. He banned Swain. Our adc first said that he demands someone to swap with him. Then sweared and asked why did he ban Swain. After he picked Mundo and said that he is the top lane now. Despite 4 of us reporting him I had to dodge the game. I wanted to send a ticket but it asks for a name.

In the ticket form put your own Riot ID (or an obviously fake one) and let the agent know what happened in the main message.
