[2021] Hard-core Ironmen of Runescape, what were the causes of your deaths?

At 1976 total skill, Combat level 111 and QP 370 I have used my last Divine Coin. I'm on my final life as a Hardcore Ironman. And, feeling salty at myself for my recent failing I wanted to hear from all of you, what killed you and why?

My first Divine coin was handed over to the Reaper at the hands of a PKer whilst doing my Wilderness Achievement Diaries. I gave him no drops or rewards, but that was of little comfort. I was working the second to last task of my Hard Diary, I had been so careful, I was almost there! And then out of nowhere, he got me.

I managed to get the money and skills I needed for the second coin. I was feeling c*cky. I was doing Dishonor Among Thieves and during the boss fight, I misunderstood the mechanics. Not only did I run AWAY from him into the shadows, by the time I realized my error he had stunned me. It was over before I knew it.

My own hubris cost me my second coin. I underprepared. Didn't research the quest like I did for my Master and Grandmaster quests and in doing so lost my final lifeline. I will inevitably die my final death and become a regular Iron Man. It is only a matter of when and to what.
