On today's SOTG, the issue w/ Negotiator's gear in the IH raid was mentioned as being looked at. Also, an issue with Morozova's rocket attack coming without proper warning. Finally, the leaderboard issue (where people cheese the raid to get a quick completion time). IH raid issues that were not mentioned, and I think have not yet been acknowledged publicly are listed below...
SOUND GLITCHES (sound will completely go out; alarm/gun/enemy sounds) during the raid. This is, by far, the most common bug we experience, as it happens to multiple people in every single raid run... Especially at boss3 (the most difficult encounter in the IH raid), where sounds are of the utmost importance. The longer you play in a particular raid session (wipe several times, etc.), the more likely the sound issues will present themselves. Closing the game and starting over will bring them back. The issue is widespread, as anyone spending time in IH raid has experienced it, and it's extremely easy to reproduce. Of course, to remedy it is a good deal of wasted time, and this adds needless length to raid completion times. Wasting players' time is never good.
USING CRANE KEY TO SHOOT THE 3 POWER BOXES LEAVES MIDDLE LIGHT AS RED. My team has now run into this twice in a row, then things worked OK for a couple of runs, but then we had it happen again. We took video from each person's perspective, and watched to confirm the power boxes were all destroyed... But still the center light remained red, so we could not get the IH key. We first thought this was yet another issue related to Negotiator's gear, but yesterday it occurred again, with no one using that gear. Our team has US, UK, and AU people, and now I am wondering if perhaps this is related to bad connections from AU players? No idea, but we will try to do it without their shooting any boxes. Obviously, a missed key is extremely frustrating, as there is no way to try again, except to wipe the raid progress and start over.
== BOSS2 ==
- Even when shooting the correct power boxes, the % progress will often glitch out and fail to progress. We have heard this can be caused by shooting boxes with exotic weapons, but yesterday everyone said they were not using exotics to shoot them, so I am not sure. Anyway, the only fix is to restart the encounter.
- Carrying RPG from one side to the other (ex: from A to C) can cause RPGs to stop spawning on that side. This causes a wipe, because eventually you are unable to stop the cannon from firing.
== BOSS3 ==
- Aforementioned sound glitches
- Getting revived (possibly, only via hive) during the encounter can cause the chains to no longer function for you. We have run into this a few times. Seems that if you have died and been revived during the encounter, there is a chance that the chains will malfunction and not take you upstairs. Instead, you will see the cut scene of the chain moving up, but your character cannot go up either chain.
- After the proper pressure bars have been adjusted (they turn blue, then we advance to next screen), sometimes they will glitch and suddenly no longer be correct. Once they are correct and the screen is advanced, they should effectively be locked so they cannot be messed up. We don't know what is happening to mess them up, as we have no people shooting them after they are already correct -- maybe an enemy bullet/explosion is doing it?

I do not know of any bugs that have not already been mentioned here and elsewhere.
