The confirmation UI stated that your upgraded weapon would have radiation damage and you would lose your 5 forma in the process.

Our current theory is that this issue only occurs if you load into Orb Vallis directly and not if you select the bounty in Fortuna and enter Orb Vallis through the elevator.

Do you recall if an allied Lich spawned in the mission when the issue occured?
