More than likely what is occurring and I have seen this, is a sub is at periscope depth, you are within spotting range of him, he is not of you, you fired and they spotted you. If a ship dives below periscope depth and NO ONE else has line of sight, the SS will NOT spot you unless he gets pretty darn close.
I have been working a lot and experimenting with this quite a bit. As a DD main I tend to be a bit more sneaky than most. Just this morning I had a CV pop up right in front of me at 5kms because I was running deep. And in another battle a BB pop up at 8kms from me because he was pushing and then fired his guns and someone else spotted him.
Everyone is still learning the various mechanics. It will take some time.
