I think it's been like this for a couple of days now but I only just noticed. My helmet is invisible without activating that option, and it happens to every helmet, not just the one I usually wear, which is transmog'd.

I'm sorry to hear that you experienced an issue with the headgear not appearing. Just to clarify the issue was resolved after you removed the transmog for a particular helmet? Therefore allowing all helmets to appear as normal. And that this issue occured despite wearing a helmet without transmog?
You also mentioned an issue with the cape. When you put the hood up did it revert to its original cape rather than the transmog one? If so which cape did this occur with and are you able to provide a video to demonstrate this.
Can you also test if this occurs with other capes or if it's just that particular one.
