Can you try disabling the Steam configurations for PS4 controllers to see if that helps? You can disable them using the following steps: 

1. Open the Steam Settings

2. Click Controller

3. Click General Controller Settings

4. Uncheck the box next to PlayStation Configuration Support

Try launching Apex Legends again after that and see if you're seeing the correct icons. Let me know how it goes when you have the chance to give it a try.
