3: I tried replicating what I did and it didn't cause it, so it appears to be random.
Also the form is asking about things that I can't answer, like:
Where did the issue occur?
How many matches had you played in a row before the issue occurred?
None of the answers on the list you must select from "where did the issue occurred" pertains to begin in the Firing Range. I also have no idea if it can occur outside of the firing range.

Details about the bug:
I'm on PC. Settings says "Installed RAM: 8.00 GB", I know there was a question in the form asking about RAM. I'll do my best to answer any other questions but I don't understand computers very well, if at all.
Firing Range, solo, as Horizon.
I used her Ultimate and while inside the Ultimate's range I used her Tactical to lift myself out of it and the visual screen distortion of the Ultimate persisted after I left the Ult. It went away when I swapped Legend. It doesn't happen every time, I tried 3 times to replicate it and 0/3 worked.
