This might be related to your Steam controller settings. Can you try something for me to see if it helps? 

 

Open your Steam Settings and click on Controller. Click on General Controller Settings, then see if the button for PlayStation Configuration Support is checked. Try unchecking it, if so. After that start Apex Legends and see if you're able to see the correct controller icons.

Can you share a bit more info on your setup? Are you running the game through Steam or Origin? Do you use any third party software for your PS4 controller?

What happens if you start your PC in a Clean Boot before launching Apex Legends? If there's anything else running that might be causing this to happen that could help to narrow it down.
