I don’t care how you do it, but having an incredibly low random chance of your character tripping over would be hilarious.

Important raid moment? guild leader delivering important speech? trying to outrun a dangerous enemy? About to win a PvP tournament?

All potentially ruined, and turned into fond memories - once the momentary anger and frustration fades away.

Same goes for your mount going lame mid-gallop, and just general chances for final destination type situations - cooking something in a fire? BOOM, a wet log explodes and molten sap goes in your eye - permanently scarring your face.

The real world is full of absolute chaos, caused by dodgy equipment, freak occurrences, and all kinds of unknowns. It adds to the wonder of how we manage to stay alive!

Clearly too much of this entropic detailing would throw the game into disarray, but it has the potential to incorporate an amazing element of chance that is seldom done well.

I would call this the Leeroy Jenkins mechanic.
