With the release of v12.41, this issue is now resolved on all affected platforms.
Update With the release of the April 16 maintenance patch for PC, Mac, and Android, this issue has been resolved on PC. We’ll keep you updated as this issue is resolved on PlayStation 4 and Xbox One.
Description Players on controller may get stuck spectating with no UI if their “Ready Up” is cancelled.

Workaround Opening the Hamburger menu allows players to Return To Lobby, or opening “Match Stats” returns the options to Ready Up.
