I'm playing on PC wth XBOX controller:
If you open the inventory to drop some equip and that equip is equippable the game instead equip that item and drop on the ground the previously equipped one, i lost my weapons due this.
I've reported the bug but be careful.
EDIT: even if you just press A to select an equip from inventory the bug occurs.
