[2021] Quickslot items keep disappearing

I've got three quickslot items that keep disappearing at random: the Signet of House Vespertilium, the Rod of Mysticism, and Aroden's Wrath.

Has this happened to anybody else?

I also still have the Rod of Mysticism and Devouring Lust abilities in my quick bar despite no longer having either equipped on my character, for whatever reason. EDIT: This is apparently a glitch with the respec feature. I respecced with nothing in my quickslots and the dupes are gone.

(Already Alt+B'd both; just wanted to see if other people experienced the same.)
