The wording "deal up to 50% more damage over time, based on the amount of damage of the initial hit." make it sound like it has similar mechanic to shock or scorch. Since its a buff, does it mean the highest dmg I deal will scale the Cruelty buff? How is the threshold calculated?

At gem level 20, The Cruelty buff provides 15.85% more damage over time to skills supported by Cruelty when the hit does 0.01% of the monster's maximum life and 50% more damage over time to skills supported by Cruelty when the hit does 3.2% of the monster's maximum life.

I don't think it does anything by itself. Lifetap Support has a modifier that increases the damage of supported skills when you have the Lifetap buff.

Basically, I'm pretty sure it works almost exactly like Inspiration Support except it cares about life instead of mana, and the spending/buff comes all in one big chunk.

Just to clarify, the Lifetap buff itself provides 20% increased life recovery from flasks at all gem levels.
