Maloney's Mechanism was released in 3.6 along with Synthesis League. Throughout the league, it dropped like any normal unique with ostensibly no ties to the synthesis mechanic whatsoever.

However, this league, it does not drop normally; rather, it only drops from things like Time-Lost Incubators.

This isn't listed anywhere on the poe wiki (including on the Synthesis League page), although it is noted on poedb.

Non-Synthesis uniques were added in 3.6.0 that aren't Synthesis-exclusive (ex. Vixen's Entrapment, Hyrri's Truth).

As far as I can remember, there hasn't been a League unique that was completely disconnected from the League mechanic since Torment. It just seems like an oversight that this one item, completely disconnected from the league's mechanic, is for some reason exclusive to its introduction league.

We've had a number of uniques that simply required the league to be active but wasn't tied to the league's mechanics in the past. Maloney's Mechanism is one such unique, and it isn't the only Synthesis-exclusive unique that is unrelated to the League mechanic. Both Hyrri's Truth and March of the Legion are Synthesis-specific too.

We've also had league-specific uniques like this a few times in the last couple years, though not as often as I'd like. For example, Delve had a few league-specific uniques that were world drops and disconnected from the Mine as well.

Anyway, this isn't an oversight and is very much intentional.
