How it currently works is that it picks from all of your portals at random. There is an option for your map device that means it will pick randomly from your entire set but they'll all match on your map device. So for example, for one map you might get all Infernal Portals and then for the next you might get all Demon King portals. There is interest among the team in having a list of favourites which allows you to curate your best portals to be chosen at random but I'm not sure of its feasibility at this point.
Edit: Since it's been asked a few times I'll just tack the answer on here, if you have multiple copies of the same portal it will only be counted as one for this purpose.

Then why does it show a map device with all different portals on the thumbnail?
It does both things.
