After the status data shows I took down 48 Breachers or Bloaters, I tried to take down more.
But when it became 52, the Defused achievement doesn't unlock.
And also don't know why the Japanese ver shows this achievement as
"Take down 50 Breachers, Bloaters or Smokers(スモーカー)."
There is no Smoker in this game.

We recently experienced issues with our services, and I believe this may have had an impact on unlocking that achievement. As these have been resolved, could you please check again to see if that achievement has been unlocked?
