It's because the hostage was injured by a trap you set off. It would be the same if your teammate cooked a grenade and rolled it towards the hostage. The person who initiated the blast would be responsible for any team, or in this case, hostage damage that is taken.

wouldnt that be a bit unfair for someone to possible get a ban or a tk penality for accidently running into a enemies trap?

I feel like that's where awareness and game sense comes into play. You wouldn't carelessly shoot down a hallway at an enemy if you had teammates in the way and risk injuring/killing them (I hope) and I feel the same philosophy would apply to running through doors. Gotta check to make sure it's safe! :)
