The bug you're reporting is a known issue:

• Theron Shan reappears in Relentless Ambitions Mission after being exiled/killed.

As your thread is a duplicate of the following one: Theron showing up in new story if you killed him, I'm closing yours to avoid multiple threads regarding the same subject.
