Checking now but it does appear to be only once across the whole account. Alt-3 (at Level 08) just did a MMR-only "prevent public execution" and Main Critter also got credit for it on the Field Research tally.

I used to think preventing the executions was the hardest of the activities but much easier now that they've made the hostages less aggressive and more concerned for their safety.
