On the final boss fight on Liberty Island. If you die and a group member kills Aaron Keener before reviving you, you respawn at the start of the mission in the Ferry Terminal.
This means you are unable to fast travel back to the mission end point to pick up the loot from Mr Keener. As the playable area for this mission is not visible on the map.
As a work around, leaving the group and then rejoining does teleport you back to the Missile Launch site to retreive your loot. Provided the group leader does NOT leave the mission area. Alternatively revive your teammates before killing Mr Keener.
