[2019] What is considered Downed?

Is it going to your knees or being knocked unconscious? I was under the impression it was when you were knocked to your knees, but I just got the "Jefferson Plaza Teamwork Merit" commendation (Complete on hard or above in a group of 4 without anyone being downed) with a PUG group. And 2 of us had gone to our knees during the final fight. Just trying to get some clarification, as I've done other Hard Missions with people going to their knees and didn't get the commendations for those maps. Though I stopped paying attention when someone went to their knees since i thought the challenge was null then.
