Can you start by checking if you've set a custom layout in the Ease of Access menu? That's found by pressing your Xbox Guide button, and going to Ease of Access > Controller > Button Remapping. If there is a custom layout there, please reset it temporarily for testing.
If the issue persists, please check our troubleshooting guide. In particular from the link, please make sure you've fully updated the console's system and all software, you've disconnected it from the network for a few minutes to reset its connection, and give it a full power-cycle.
If none of those steps solves the issue, please let me know - I'll have some further ideas we can try to get this resolved for you. In that case, it'd be really helpful if you could explain what 'the usual fixes' are, so I don't suggest the same ones you've already tried. Thanks
