In the WONY expansion there's an area near the Waterfront CP where a BT execution is happening every time you log on. Near the center there's a special ammo crate (high velocity rounds) that if you approach from the rear (of the crate) and press the Loot button...well, see for yourself what happens.
I realize in the scope of game-killing bugs the Div 2 has this is small potatoes and barely rates as an inconvenience, but I find it hilarious and indicative of the quality control (or lack thereof) this game has.
P.S. It's quite repeatable, too.
