In the missions, The Tombs. Playing on higher difficulty (making rogue encounters more frequent). The objective "Search the infirmary." A rogue encounter can happen and after the encounter is dealt with, you will be progression locked from finishing the mission. This has happened to me many times and happens when rouges encounter in the mission.
