I mean, I recognize people on that list that aren't actually playing, but just sitting logged into their servers all day.

There are other things to consider as well, like my profile only shows that I have 5,932 hrs on record. However it isn't recording any of the hours I had playing before Steam started tracking that, any "offline" hours from before they started including those, or any of the hours I have spent testing closed builds. Even adding all of those in, I wouldn't put myself past probably 8 or 9k hours. Definitely not as many hours as I played Skyrim, but again Steam falls short with only showing 1217 hours because for the longest time I couldn't launch skse through Steam.

OP did ask for who has the most hours, and I simply provided a curated list derived from public Steam profiles. If a profile is private, they won't be on the list.

Currently it does count. And it has counted since sometime around 2017 off the top of my head. Prior to that though Steam wasn't tracking playtime if you were offline.
