So this happens randomly & it's not a huge inconvenience but I figure I should notify you guys. It's happened at least twice where my primary (iridescent axe) & secondary (icewall) switch hands for no reason. It happens when I load into my saved games, ive never had it happen midplay (as of yet).
also wondering if anybody else experiencing this?

I couldn't find any reports of this issue so far. Could you please provide us with a short video showing the weapons and then loading a game to see if they swapped places? It would greatly help us to understand the problem and report it to the production.
Do you have the Dual Swap skill unlocked? Sometimes it's easy to trigger it accidentally in combat, so please make sure that it's not the case here 
Thanks for the warning @azullFR ! Indeed, some players reported losing weapons while using the skill with a torch in the past, but we haven't received any new reports in a long time, so I think it's safe to say that the fix worked
