Preordered this edition that came with the Berserker DLC pack. Redeemed the code and got an email saying instructions will be sent to you via email. Have still not received my email!

I've checked both of your accounts, and as it appears that the entitlement for the bonus is present on both, I'm assuming that this issue was since resolved for you, which is great!
