All the fancy settings of TMPE aren't going to help you unless you understand the basics of how vehicles travel, so let's start there. They pick a route much like if you put a starting point and a destination into a GPS. They only change lane when they need to, which is usually for turns.
When you see a lane not used, it's because it doesn't go anywhere useful - they don't need to turn into it, so they don't. Best way to illustrate how routes are picked is this image of two similar, but slightly different roads. The lines represent any route vehicles might take. One has a lot of wasted lanes, the other sees much more use, because there are turn-only lanes.
