Visit https://www.pathofexile.com/my-account/connections and click the "Remove" button in the Twitch section to begin the process.

These are bundles from Twitch Prime that required you to visit the Twitch page on your account here in order to finish redemption. Even if you didn't check that page you must have still been entitled to the bundle so it resolved when you visited it just now.

Yep you're good. If you have a blank page or some old Twitch rewards showing up it's a good sign!
