That is correct. When I do fractals, I don't put up the tag until the last boss is at like 5% health, and then I take it back down when the Transit Query comes up.
Needs at least 3 health bars, 2 aren't enough.

Your best bet is to hang around in one of the cities at reset, a lot of people offer theirs around that time.
especially if that city is in the region for the gathering daily (eg. Hoelbrak when Shiverpeaks Miner is one of the dailies)

Auto-refresh as in it's 100% passive so long as it's in inventory? Also, just to be certain, this is boosting defense specifically, not toughness? Hoping so since that'll ideally mean it doesn't mess with aggro in certain encounters.
yeah, the original design was toughness, but we didn't want the toughness tank to have the buff expire and then lose aggro, so we changed it to defense.

It does work in WvW, but it's a lower amount.

be aware that some skills are slightly different between game modes. The Golem in PvP should give you a general sense of how the skill feels, but don't rely on it for specifics of what the skill does in other game modes.

In the options menu, you can also set the Content Guide to Disable Personal Story, and it will point you to nearby events and map completion objectives. If you don't want events either, there's an option for that too. I am constantly switching between these three settings depending on what I'm trying to do.
