The NPC in the building never spawned. It's the only mystery left in London and so can't complete the region and get achievement.
I was able to move all jars and obstacles to create a path, but there is no NPC to start/finish the quest.

I am also sorry to hear that you are unable to complete the mystery 'The Demon Odour at the Tithe' as the NPC has not spawned.
I can confirm that this issue was reported in 2021 and was fixed with update 1.2.0.1. We have not received any other reports of this issue since then.
