This has been happening to a few others and myself.
Out of the 6 times it happened to me before I started using other grenades, they were all clearing spores.
Different maps, ops, difficulties - so one can only assume it's something to do with stun + spore. Hope it gets fixed soon!

It’s been hard to reproduce as it seems to just be random, but for your third question, yes, it does revert to default settings.
This includes the menu narration, (I turned that on by mistake when first launching the game, maybe it’s related? No idea) anyways, I know it’s reset everything when I load and the menu is talking to me.
It resets sensitivity and stuff too so yeah, default settings

A friend and I were playing last night and couldn't complete a single area on the easiest difficulty. My friend got frustrated and quit and I though "why not try one more time solo?". running solo it was at least 10x easier than in a duo i was able to get all the MIA people back and complete all the objectives multiple times. so is there only solo and full squad as far as the scaling goes?
No, the scaling applies to Duos as well. If you need to destroy 15 nests in Trios, you need to destroy 10 in Duos & 5 in Solos

Glad you're liking it! There's a load of useful combos and some Ops/Gadgets that might not seem as useful at first glance, can actually be super useful!
So if I'm understanding you correctly, Capitao should have his default skin from Siege, and that this is a wrong skin being visualized?
Once you get an Op to level 10 the OPERATOR LEVEL is secure. If you fail to rescue them they will not go to level 9 for example, but you do lose a certain amount of XP towards Milestone progression.
Someone will probably lawyer me on that but I'm fairly sure that's how it works.
