I want to report another similar problem,
when looking to save an icon from a custom block,
the icon is not being updated...
I believe that capturing block icons and block macros is broken.

If you haven't already, a reinstall could be helpful! I would also advise looking through the troubleshooting steps here and give any of these a try that you haven't done yet! These steps can help resolve a lot of different issues, but if it doesn't help the issue you're seeing it gives us a better idea of what to look for.

Thank you both for the update! I'm really sorry to hear this issue is persisting.

Before I report this to the team, I just want to clarify one other thing. Does this specifically prevent the creation of new icons? Or does it break old ones as well?

It seems from the picture that @milksh2ke has some macros with images but others are just generically assigned "macro075" etc., so are old icons still good to go and new ones can't get icons? Or is it random what macros have icons and which don't?
