I'm sorry to hear you're having issues!
The matter regarding the 'Failed to Save' message is currently being looked into by the team. If you could please confirm the following for us to pass on it'd be appreciated;
- Do you use Quick Resume/Switcher? Do you leave the game minimized at all, and if so, how long?
- How often does the error appear?
- If you close the app and re-launch it, are you able to save again? Please specify for Quick, Auto, and Manual saves:
- Does this issue only occur while playing the expansions content or do you also experience it on other maps in the game?
- How much space do you have on your console's storage?
- How many saves slots do you have filled? And does this issue happen on all of them?
- Are you able to create a new manual save (not overwriting your existing one)?
As for the trophies, I'm afraid that when moving between consoles (PS4 > PS5), trophy activity is not transferred over too and the requirements will need to be re-done for them to trigger. In many cases such as collect X amount of an item, collecting one should re-trigger the trophy to pop but as the trophy system is operated via Sony's systems, we don't have access and cannot force these trophies for you.
