i've know of the project for a time now via things like job adverts for a "new survival game" and the such, and as a huge survival game enjoyer and runescape addict i'm really interested to see what Jagex can bring to the table, i think the only thing that actually beats my runescape playtime is my survival genre playtime in total.
here's hoping i get in, i'd like to think i know what makes a decent survival game given i've played well over 20k hours of them, i'd love to have the opportunity to help shape one with my feedback.

If it's closer in specifications to a, say, 2070, then feel free to drop that in instead. It's a way for us to get a rough aggregate/estimate of people's hardware and what we're testing for, here!

If I submitted a response in the first 5 minutes of it being out, just leaving off the last digit, should I fill it out again with the full ID, or notify someone if contacted by email? I don't want to be disqualified by submitting multiple entries.
You won't be disqualified, don't worry - and also, don't worry about being early to signing up! We aren't selecting testers by who completed the form fastest. I really appreciate you being so eager to participate. It means a lot.

Absolutely not replacing anything, it will be another option to play alongside the MMO games.
Change went out earlier on, you are given the bones you have the least of now.
While I know Jagex is capable of creating a great game, I have a sour taste in my mouth of a studio that developed a skillset on this ancient game and engine, making a modern game. Interest is peaked though
This is a really good point, genuinely. Be assured that the large majority of devs working on this project have huge, extensive industry and tech experience outside of Jagex. There are one or two idiots like me working on it too, but I've modernised my knowledge, I promise.

The select menu for GPU model is missing at least a few items. I have a GTX 1080 TI and an RX 6700 XT, but the only options were the base GTX 1080 and RX 6700, which are materially different GPUs. I went ahead and selected the closest match, but the lack of specificity likely isn't ideal for data collection.

We initially had an exhaustive list (that was so long that it was impossible to scroll through) but we've bracketed the GPUs and CPUs into their nearest serial point - If you have a 1080 Ti, then select 1080, and if you have an RX 6700 XT, please select the RX 6700. They're different, but close enough that we have an idea of the kind of hardware you have, which is all we'd need to know for now. Thank you!
