Can you ensure her console is on a wired connection, as well as yours?
Where is the last place she logged out from?
Is there a high amount of buildings, placeables, thralls and/or light sources in that area?

When followers return home, they simply end up going back to their previous guarding spot after a while, and depending on where that is, it’s possible that it could have spawned inside certain structures, such as base walls. Are you certain you haven’t set some of them to guard in a different place other than your base?
As you’re on a server you own, are you able to set yourself to admin and look under the terrain and building pieces to determine if they could’ve clipped through them?
That being said, we’re focusing on addressing thrall related issues and bring some quality-of-life improvements to them in the upcoming patches, which should prevent these occurrences and assist in managing your followers.

Could you also provide more details regarding being unable to respawn?
Were you stuck in the loading screen or were you unable to select a location to spawn in?
Are you able to currently login to the server?
Hello @Kahdel, our server reporting form is meant for server issues such as downtime and unusual performance, which are relayed to G-Portal once confirmed. For gameplay issues it’s best to reach out to us through these forums.
Regarding this issue, did you check the decay timer for the building pieces to ensure that it has been refreshing properly?

Hello @Melleah, when one player builds at the edge of another’s claim, there will be a claim intersection area where both players shouldn’t be able to build in, which is intended and expected behavior for the current claim system.
As a workaround, most players extend their claims beyond their main buildings through the use of walls or other placeables.

Also, if and whenever you do get crash report prompts, always send them through the PS dashboard once prompted so our team can get a hold of them and evaluate the causes.
