Map Positioning can often take two forms:
You can play frontline-esque and support an allied Destroyer. You can use your surface detection glimpse consumable to help spot/ping enemy DD's that use smoke while relying on your DD to prevent anyone from rushing your position.
You can play a backline-esque role where you act in a more independant fashion to try and surprise enemies.
There is a commander skill which gives bonus damage if you have a double-ping before you launch a torpedo, but outside of that it's personal preference.
You can launch to allow your torpedoes to spread out, then ping so they converge from multiple angles.

Our client does have Voice Chat capability. It can be enabled in the Sound part of the settings menu.

Service Cost is absolutely a factor for consideration. Premium Ships not only have a reduced service cost but also have a built in bonus to Credits earned, so it's not surprising to see the Atlanta pulling so far ahead of the tech tree Lightning.
Operations can be played at lower then Tier 8. While you won't have the same damage expectation, you will have an advantage from the reduced Service Cost which aids in rewards.

Your Base XP being between 1100-1600 allows bonuses used to have more value per single use than in a Co-Op, but in a less stressful environment than PvP. PvP continues to be the area of highest-possible return, but it is a wildly unpredictable situation unlike Operations which provide scripted, albeit challenging content.
Probably best to provide the Replay?
Homing Torps tend to arm about .5km away, and Dumbfire Torps are around 1km. The ranges can vary a bit, but that's a general rule of thumb.

There is more to the Detonation mechanic than a simple die-roll. The Magazines are modules that exist in the ship. Damage has to be dealt to the magazines in order to cause a detonation chance, so shell placement and type can and do affect the situation.
Someone else mentioned that you had 20 Detonations though show nearly 5k battles. I did look up my Free-to-Play account and found I have a single Detonation out of 283 Random Battles. While this can certainly be a tide-turning event, it is not as simple as a 1-in-10 chance to explode for each shell hit.
Hits must be accurate in that they must interact with the magazine, and further the magazine needs to be damage sufficiently to allow for a chance of Detonation at all.

I did make and use a Free-to-Play account that I streamed on for a while. I never used Det Flags in Randoms so I would always use the Slot 1 Upgrade that reduces Detonation Chance. It really does work quite well.
If this was the case, DCP would not be able to repair incapacitated Engines/Rudders/Weapons?
I've already made a suggestion quite a while ago about having something to help tell the difference <3 This was back when I was a CC.
My thoughts were something like having a Wifi Signal icon above the torp while it was homing-active. A player would be able to identify quickly if the torps were dumb-fire or not. Also, the icon could disappear when the homing disables as the torps go dumbfire due to proximity to target.

I do believe the wavy particle effect on homing torps is our solution for helping players identify homing vs non-homing, though I don't believe the particle effect disappears when the torpedo "goes ballistic" for the final bit of distance. I feel that would be useful in helping players understand the homing mechanics and would love if that could be implemented, but no idea on if that'll be a thing.
You said that you crashed and had a lot of trouble getting back into the match. I believe you said it took 6-7 minutes, which is a significant amount of time. The time itself is the most likely factor in the Inactivity sanction so there's no reason to request a Replay. Still, if you have it than I'm not opposed to looking it over.

This thread's OP reported no crash/disconnect, so having a Replay would allow us to find why the system tripped in this case. It sounds like he went functionally afk after the bots that were available were dead (because Two Brothers is a large map so it's hard to get to other bots) and the system noted that and actioned it. It would be useful to see the situation to gather specifics.

I've been contacted by 3 people which have raised their concern with repeatedly playing the same Operation when using the Random Operation button. At this point, I don't have a lot of information that this is a widespread or non-random event.
If anyone does have experiences where they are playing the same Operation repeatedly during a play session of Random Operations, please provide a screenshot of your match history in this thread.
If you are on the surface, an enemy sub can go to periscope depth and activate their surface detection consumable. It updates the minimap and gives brief "ghost" impressions of enemy ship positions.
It's possible that you are detected in that manner and pinged while holding a straight course.
