There’s an issue in which thralls can become unresponsive if equipped with a weapon kind different to the one it spawned with then broken at the wheel of pain.
That could explain the issue you reported.

Hey there,
G-Portal is looking into it.
Hey there,
We’ve contacted G-portal about it.
For server issues, please contact G-Portal through their normal communication channels.

Please also run a search before posting about an issue just in case somebody posted about it recently.
Thanks.
Hey there,
We’re aware of this issue. The shield is not gone per se, but goes to a thrall “invisible” inventory for a while. It should reappear in the normal inventory after a while.
Our team is looking into it.
Thanks for your feedback.
Hey everyone!
We’ve rolled out a patchfor the game that contains the new DLC pack: Treasures of Turan, fixes for UI bugs, chests unlocking after server restart, and also adds a privacy setting for the Event Log.
Please remember that updates can mess with your installed mods. We suggest taking a backup of your current database before updating to any new patch if you have mods installed.

Hey @WaltZie
Have you checked the event log and see what could’ve happened?
Hey there @RabidChimp805
Could you try moving away from the region you’re at and coming back after being away from it for a few minutes and see if that fixes the issue?
Hey @Smokedgouda94
Welcome to our community and thanks for sharing your feedback with us. We’re sending it to our team so they can take it into consideration.
Hey @Kumiko
Thanks for sharing your findings with us. We’ve sent your tip to our team and hopefully it’ll help them crack this egg once and for all
Additionally to what @Kwalya mentioned, if there’s suspicions of DDoSing, make sure also to get in contact with G-Portal and/or Sony.
To clear any confusion, we don’t have server admins on official servers, nor any admin password.
Sending a suggestion to our team to see if they can add such an option in the server browser
It’s random, after a certain period of time. Problem is, on dedicated servers this isn’t much of an issue as they run pretty much all the time, but for a listen server you have to be playing for the server to be running AND you have to be around that area, thus making those showers a little more complicated to show up.
For the time being the setting is disabled once the server is started as messing around with that setting after the server file is created could cause a lot of issues afterwards. Private server admins could fiddle with it, but it’s not something we would recommend.
Hey @Andyfrom93
Also make sure to learn the feats required to craft the new items for this DLC. You can see the requirements by going to the Feats tab and selecting any of the new Turan pieces.
Ok, that’s a good start.
If you resize the log output of the Dedicated Server Launcher so you can see the log very well, if you try to connect to the server, do you see anything happening on the server at all? Can you see the logging happening, and do you see any error reported by the server (mod errors, time out message, network problem, etc…).
Is it a VM on your own network, or in some hosting center somewhere else?
If it’s somewhere else, from your client machine, can you try to check the network stability by running a continuous ping for a few minutes, example:
> ping /t you.server.ip.here

I see that there was some references to this particular error in some other thread:
The servers are coming down for a small fix-em-up patch April 30th at 9:00 AM EDT / 13:00 UTC. Downtime ETA is ~2 hours.
Hey there @steve2para
Could you check your event log and see if there is anything else going on that could explain these disappearances?
