As expected, there is a ton of information that you will want to know and we’ve compiled this mega-thread to cover as many topics as possible.
I think you are using the death protection too heavily and not realising when it's not active. For instance any hit that would kill you leaves you at like 10% hp but then that goes on like a 30sec cooldown and during that time you can die instantly. There is 2-3 death prevention methods to help keep us alive cant remember specifics as I saw them awhile ago but just check a wiki for it.

My understanding is we can now (at last) play through the entire game on the highest challenge tier?
Correct - You can, for example, start a level 30 boosted character and play through Worldslayer, level its Apocalypse Tiers as far as you can get, then return to the original's campaign starting point and play through the campaign at a difficulty level matched to you and your gear.
