If that does not help, could you backup your current save and fully wipe the game’s data before proceeding with a clean reinstall?
Also, do you have the game installed to the console’s internal drive?
If that’s the case, would it be possible to try installing it to a different one, such as an external drive?

Should any of you be able to login outside of that area, are you able to approach it and have it load without issue?

Having a PS4 Pro and the game installed to a SSD or external drive definitely helps with performance and loading the game/assets, but we’d suggest that you first try to rebuild the PS4 database and, if that doesn’t help, try and perform a clean reinstall of the game. Hey @KarstenZ1971, knocked out thralls are intentionally vulnerable, you need to be additionaly careful if you want to drag them over great distances.
As a possible workaround, you may choose to build outposts with Wheels of Pain closer to the areas you usually grind for thralls rather than dragging them back to your main base.

It’s possible that something is wrong with the savegame/database as this is not a common occurrence, which could happen due to disk errors or game crashes, for example. Should that be the case, it won’t be solvable by patching, only by restoring an earlier save or starting a new game.

Did you try toggling the Show Name Plates setting off, restarting the game, then toggling it on again?
Also, if you have more than one controller plugged into the console, could you please try restarting the game while having only one controller?
