So are there no more tiers/levels of perks? Is it just one level of each perk?
Still tiers but the number of tiers for some of the perks has been reduced mostly based on having nice upgrade steps. Also with most perks the secondary effects (if any) are now available at tier 1.

What happens if the last player to go down hasn't used it yet? Is there a grace period for them to activate it or does the game just end?
If any of the players has an unused Iron Will there is a grace period yes.
How would limited-use perks work in Deep Dives? For example, if Iron Will can only be activated once, is that once per stage, or once for the entire dive?
It's all per stage.
Also would be cool if dwarfs when typing put their hand to ear like they holding earpiece, or something like that to show that player chatting.
