It looks like you could upgrade to Windows 10 build 1809. Search "Windows Updates" in the desktop search bar to check for pending updates. Your video card driver is from 2018 and when they are outdated they can cause technical issues, so I'm heavily inclined to lean towards that as to what's causing this. Your PC specs are around the minimum requirements to run this game, so I would try playing at lower video settings and try borderless window to see if that helps boost performance. Try rebooting your network and power cycle the console. Let us know if you're still getting that penalty. It should be disabled for everyone.
Reboot the power to the modem/router
Are you able to record some clips of this happening so we can see how it's not showing up on your screen? Are you waiting until your whole squad is eliminated before going back to the menu?

Right-click on the game before you launch it and select "Repair."
Try rebooting your network. If you're familiar with port forwarding we have the port numbers listed for Apex Legends below. Try rebooting the network first though to see if a simple refresh of the connection addresses this.
